Abstract [sv]
Bakgrund: Samtal om palliativ vård har som syfte att förbereda både patienten och de närstående inför framtiden. En av vårdpersonalens uppgifter är att förmedla svåra beslut om övergången från kurativ till palliativ behandling. Specialistsjuksköterskan bör arbeta evidensbaserat och i ett team med andra specialister för att tillgodose patientens och de närståendes vårdbehov. En god kommunikation mellan de närstående och vårdpersonalen är avgörande när det gäller förberedelser för slutet av patientens liv. Det behövs kunskap om de närståendes perspektiv på samtal om palliativ vård, därför är det viktigt att undersöka dessa upplevelser vidare.
Syfte: Att belysa hur närstående upplever samtal om palliativ vård med den onkologiska vårdpersonalen i samband med obotlig cancer.
Metod: En litteraturöversikt. Kvalitativ design med induktiv ansats.
Resultat: Resultat redovisades i fem teman: att förneka verkligheten, att känna missnöje i kontakt med den onkologiska vårdpersonalen, att sakna information och kunskap, att förstå den svåra situationen och att känna trygghet i kontakt med den onkologiska vårdpersonalen.
Slutsats: Denna litteraturöversikt belyser att samtal om palliativ vård under sjukdomens förlopp är viktig för implementering av palliativ vård. Dessa samtal är dock ofta svåra upplevelser för de närstående. Tillgång till information och kunskap i den mån det är önskat av de närstående samt den onkologiska vårdpersonalens lyhördhet minskar de närståendes ångest och hjälper dem att acceptera besked om tillämpning av palliativ vård.
Abstract [en]
Background: Conversations about palliative care aim for preparing both the patient and the family caregivers for the future. One of the tasks of healthcare professionals is to communicate difficult decisions about the transition from curative to palliative treatment. The specialized nurse should work in an evidence-based way and in a team with other trained specialists to meet the patients and the family caregivers needs. Good communication between the family caregivers and the healthcare professionals is crucial when it comes to preparing for the end of the patient’s life. There is a need of knowledge about the family caregivers’ perspectives on conversations about palliative care which is why it is important to study these experiences further.
Aim: To highlight family caregivers’ experiences of conversations about palliative care with the oncology professionals in relation to an incurable cancer.
Method: Literature review. Qualitative design with inductive approach.
Results: Results were presented in five themes: to deny reality, to feel dissatisfaction while in contact with the oncology professionals, to lack information and knowledge, to understand this difficult situation and to feel safe while in contact with the oncology professionals.
Conclusion: This literature review highlights that conversations about palliative care during the course of the illness are important for the implementation of palliative care. However, these conversations are often difficult experiences for family caregivers. Access to information and knowledge to the extent requested by the family caregivers along with the responsiveness of the oncology professionals reduces anxiety and helps with accepting the transfer from curative to palliative care.
