Abstract [sv]

Bakgrund: Den globala andelen äldre ökar, vilket ställer högre krav på en individanpassad och kvalitativ hälso- och sjukvård. Personcentrerad vård har lyfts fram som en viktig vårdmodell för att främja hälsa och välbefinnande hos äldre personer. Trots ökad kunskap om personcentrerad vård kvarstår utmaningar i dess praktiska tillämpning.

Syfte: Syftet med denna litteraturöversikt var att belysa äldre patienters erfarenheter av personcentrerad vård.

Metod: En strukturerad litteraturöversikt genomfördes där kvalitativa, kvantitativa och mixade metoder inkluderades. Totalt analyserades tio vetenskapligt granskade artiklar, publicerade mellan 2015 och 2025, hämtade från databaserna CINAHL Complete och PubMed. Tematisk analys användes.     

Resultat: Två huvudteman identifierades: (1) Personcentrerad vård i praktiken, samt (2) Vårdens organisering, tillgång och upplevd kvalitet. Resultatet visade att patienter värdesatte att bli sedda som individer, uppleva respektfull kommunikation och få kontinuitet i vården. Hinder för personcentrering var bristande information, begränsad delaktighet och organisatoriska brister.

Slutsats: Personcentrerad vård bidrar till ökad autonomi, trygghet och värdighet för äldre patienter. Samtidigt kvarstår ett glapp mellan teori och praktik. Sjuksköterskor och vårdpersonal behöver integrera ett personcentrerat förhållningssätt i både kommunikation och vårdplanering samt aktivt inkludera patienter och deras närstående.                                                          

Abstract [en]

Background: As the global population ages, the demand for individualized and high-quality healthcare increases. Person-centred care has been emphasized as a key approach to enhance health and well-being among older adults. Despite increased knowledge about patient-centered care, challenges remain in its practical implementation.

Aim: The aim of this literature review was to highlight older patients experiences of person-centred care.

Method: A structured literature review was conducted using both qualitative and mixed-method studies published between 2015 and 2025. Ten peer-reviewed articles were included, identified through searches in CINAHL Complete and PubMed, and analysed thematically.

Results: Two main themes emerged: (1) person-centered care in practice, including participation, respect for patient wishes, and involvement of relatives; and (2) Healthcare organisation, access, and perceived quality. Findings showed that patients appreciated being treated as individuals, valued respectful communication, and emphasized the importance of continuity and inclusion in decision-making. Barriers to PCC included inadequate information, lack of involvement, and organisational limitations.

Conclusion: Person-centred care contributes positively to older patients' sense of autonomy, safety, and dignity. However, discrepancies between theory and practice remain. To meet the complex needs of elderly patients, healthcare providers must integrate PCC as an ethical and practical approach and involve patients and relatives in meaningful ways.
