Abstract [sv]

Bakgrund: Palliativ vård syftar till att lindra lidande och öka livskvaliteten för personer med obotlig sjukdom. Vården präglas av ett helhetsperspektiv där fysiska, psykiska, sociala och existentiella behov beaktas. Den ges på olika vårdformer och bygger på samverkan mellan olika professioner, patienter och deras närstående. Sjuksköterskor har en ett ansvar att ge personcentrerad omvårdnad, främja trygghet genom att möta existentiella frågor med empati och närvaro.

Syfte: Att beskriva patienters erfarenheter av existentiellt lidande inom palliativ vård.

Metod: En litteraturöversikt baserad på åtta vetenskapliga kvalitativa originalartiklar, som identifierades via systematiska sökningar i databaserna CINHAL Complete och PubMed. Artiklarnas resultat tematiserades baserat på skillnader och likheter.

Resultat: Dataanalysen resulterade i tre teman: Förlust av identitet och livskvalitet, Existentiell ensamhet och utanförskap samt Oro inför döden och meningsskapande.

Slutsats: Existentiellt lidande är en central del av patientens upplevelse i palliativ vård och förknippas ofta med identitetsförlust, ensamhet, utanförskap och oro inför döden. För att lindra detta lidande krävs en helhetssyn där patientens värdighet bevaras och det existentiella behovet erkänns och får komma till uttryck Att våga möta det existentiella lidandet är inte bara en professionell uppgift utan ett uttryck för genuin omsorg och respekt för människans djupaste livsfrågor.

Abstract [en]

Background: Palliative care is intended to alleviate suffering and improve the quality of life for individuals living with incurable diseases. It is grounded in a holistic approach that considers physical, psychological, social, and existential dimensions of care. Palliative care is delivered across various healthcare settings and is based on interdisciplinary collaboration between healthcare professionals, patients, and their families. Nurses hold a central role in providing personal-centered care and are responsible for fostering a sense of security by meeting existential concerns with empathy and presence.

Aim:To describe patients' experiences of existential suffering in palliative care.

Method: A literature review based on eight scientific qualitative original articles that were identified through a systematic search from the databases CINHAL Complete and PubMed. The results of the articles were thematically analyzed based on differences and similarities.

Results: The data analysis resulted into three themes: Loss of identity and quality of life, Existential loneliness and exclusion, and Anxiety about death and Meaning- making.

Conclusion: Existential suffering is a central aspect of the patient's experience in palliative care. It is often associated with loss of identity, feelings of loneliness, social exclusion, and anxiety about death. Spirituality, life reflection, and close relationships emerged as important factors to manage existential suffering. Alleviating existential distress requires a holistic approach to preserve the patient's dignity and acknowledge their existential needs. Confronting existential suffering is not only a professional task, but also an expression of genuine compassion and respect for the deepest questions of human existence.
