Abstract [sv]

Bakgrund: Transpersoner tillhör en minoritetsgrupp som genom åren blivit utsatt för diskriminering på olika nivåer i samhället. Detta leder till en ökad risk för att utveckla psykisk ohälsa som depression och ångest. Primärvård är hälso- och sjukvårdstjänster som är allmänna och inte begränsade till vissa sjukdomar eller patientgrupper. Enligt svensk lag och Agenda 2030 har alla individer rätt till jämlik vård oavsett könsidentitet. 

Syfte: Syftet var att beskriva transpersoners upplevelser av bemötande inom primärvården. 

Metod: Litteraturöversikt valdes som metod. Litteratursökningen genomfördes i databaserna Pubmed och Cinahl Complete samt genom en manuell sökning och totalt inkluderades 10 vetenskapliga originalartiklar.

Resultat: I resultatet framkom fyra teman: Användning av fel pronomen uppfattades som kränkande, Vårdpersonalens förutfattade meningar och fördomar mot transpersoner, Avslöjande av könsidentitet förändrade vårdrelationen negativt och Kunskapsbrist hos vårdpersonal- ett hinder för att få jämlik vård.

Slutsats: Transpersoner upplever negativt bemötande inom primärvården på grund av bland annat diskriminering och okunskap. Även positivt bemötande framkom såsom respektfulla vårdmöten. För att öka transpersoners trygghet och tilliten för vården behöver sjukvården ha mer kunskap om transpersoners behov och arbeta mer inkluderande för att säkerställa jämlik primärvård.

Abstract [en]

Background: Transgender people are a minority group that has, over the years, been subjected to discrimination at various levels in society, which leads to an increased risk of mental health issues like depression and anxiety. Primary care services are general healthcare services that are not limited to specific illnesses or patient groups. According to Swedish law and Agenda 2030 all individuals have the right to equal healthcare regardless of gender identity. 

Aim: The aim was to explore transgender individuals experiences of interaction within primary healthcare. 

Method: Literature review was chosen as a method. The search for articles was carried out in the databases Pubmed and Cinahl Complete along with a manual search to conclude a total of 10 original scientific articles. 

Results: The results identified four themes: Incorrect pronouns was perceived as discriminating, Healthcare staff expresses prejudices and stereotypes against transgender people, Disclosure of gender identity negatively changed the healthcare relationship and The lack of knowledge among healthcare staff – an obstacle in equal health care. 

Conclusion: Transgender people experience negative encounters with primary healthcare personnel as a result of discrimination and lack of knowledge. Positive experiences also emerged and were about respectful healthcare encounters. To increase safety and trust in healthcare, the healthcare system needs to become more competent regarding the needs of trans individuals and work in a more inclusive manner to create an equal primary healthcare.
