Bakgrund: Hjärtsvikt är en vanligt förekommande, dödlig sjukdom med ett komplext sjukdomsförlopp som medför kraftig påverkan på livskvalitet och funktionsförmåga. Hälften av de som får en hjärtsviktsdiagnos i Sverige avlider inom fem år. Palliativ vård är symtomlindrande vård för sjukdomar utan bot och ska initieras i de senare stadierna av hjärtsvikt.

Syfte: Syftet var att beskriva sjuksköterskors erfarenheter av att vårda patienter med hjärtsvikt vid livets slut.

Metod: Allmän systematisk litteraturöversikt. Litteratursökningen utgick från databaserna PubMed och CINAHL Complete. 10 studier, varav 8 kvalitativa och 2 kvantitativa analyserades genom tematisk analys.

Resultat:Sjuksköterskornas erfarenhet innefattade att upptäcka, tolka och övervaka symtom och tecken på försämring och att ge symtomlindring. Utöver detta så lyfte de kommunikationen med patient och närstående som en central erfarenhet. Sjuksköterskorna lyfte en rad utmaningar kopplat till sina erfarenheter men också till sjukvårdens organisation, struktur och andra professioner. Sjuksköterskor upplevde det som en utmaning att förloppet vid hjärtsvikt är oberäkneligt.

Slutsats: Sjuksköterskor har en central roll vid omvårdnad av patienter med hjärtsvikt i livet slut. Det finns områden där sjuksköterskor uppfattar att vården av patienter med hjärtsvikt i livet slut är utmanande och att det finns utrymme för förbättring.

Background: Heart failure is a common and fatal disease with a complex disease progression that significantly impacts quality of life and functional ability. More than 50 percent of patients diagnosed with heart failure in Sweden die within five years. Palliative care is care alleviating symptoms without curing the underlying disease and should be initiated in the later stages of heart failure.

Aim: The aim of this study is to describe nurses' experience of caring for patients with severe heart failure at the end of life.

Method: This study is a literature review. The literature search was based on the databases PubMed and CINAHL Complete. Ten studies, including eight qualitative and two quantitative, were analyzed using thematic analysis.

Results: Nurses' experiences included monitoring and interpreting symptoms and signs of deterioration, as well as providing symptom relief. Nurses highlighted communication with patients and relatives as an important experience. Nurses noted several challenges related to their field of work including the fact that progression of heart failure is unpredictable. The organizational structure of the health care sector and involvement of other professions in healthcare also included challenges for nurses in this field. 

Conclusion: Nurses play a central role in the care of patients with heart failure at the end of life. Nurses experience that the care of patients with heart failure at the end of life as challenging, indicating that there exist areas for improvements.
